Information for our Patients

Welcome to Valley Medical Care. Here is important information you need prior to your  visit at our office.

Prior to your appointment  you will need to read our Patient Consent and Financial Information document. Click on the link below. At the time of your visit you will be asked to sign an electronic version of this document. The electronic copy will also include the financial information you will provide on a questionnaire assigned to you through your patient portal.

Patient Consent and Financial Information

If this is your first visit to our office, please arrive 20 minutes prior to your scheduled appointment time. If you have not completed the assigned questionnaires prior to your appointment, please arrive 45 minutes prior to your appointment to complete these in the office.

You will need to bring a government issued picture ID and your insurance cards to your appointment.

Patient Portal

You were assigned a user name and password at the time you scheduled your first appointment. Once you log in to the portal you will be asked to create your own unique password. You should protect this to prevent unauthorized access to your medical information.

Please log in to the Aprima Portal to complete the the following prior to your appointment:

  • Registration (demographics--personal information)
  • Questionnaires (financial information, medical history, etc.)
